Pharmacogenomic Candidacy — Pre-Screen
Intake screening aid  ·  routes a candidacy signal to the provider for review
What this is: a front-of-intake screen run on the patient's actual current and considered medications. It flags whether a genuine drug–gene candidacy signal exists and produces a summary for the ordering provider to review and decide on.
What this is not: an order, a panel selection, or a statement of medical necessity. The provider independently determines whether testing is indicated for a specific medication decision and documents that in the chart before any order is generated. Enter only medications the patient is truly taking or that the provider is genuinely considering.

Add the patient's medications to see the candidacy result.
2Diagnoses
For the MDD / neuropsychiatric panel intended-use pathway, the code must be specific (e.g., F32.1, not F32.x or F32.9). Quick-add covered codes:
3Patient-reported symptoms — for provider review
These are prompts for the provider's clinical assessment of symptoms the patient is actually reporting — not justifications for testing. Any new medication or testing decision is the provider's to make and document.
Hand this to the ordering provider through your EHR or secure message. The provider reviews, makes the clinical decision, and signs the line at the bottom — the screen does not place the order.
